Paciente de 54 años de edad que llega de emergencia por sangrado vaginal intenso de 40 minutos de duración y de presentación aguda.
El mioma uterino es el tumor benigno más frecuente en mujeres: está presente, con o sin síntomas, en una de cada tres mujeres. Se desarrolla dentro o pegado a la pared del útero. Consiste en la proliferación anormal de células musculares del útero (mioma), a veces acompañada de proliferación de fibras del tejido conectivo del útero (fibroma).
El útero es el órgano hueco, en forma de pera invertida, donde se desarrolla el feto.
El mioma uterino es una masa de tamaño variable, desde 1-2 centímetros de diámetro hasta varios kilos de peso, redondeada y de consistencia firme elástica, que aparece dentro del útero. Puede ser único o múltiple. Crece, en parte, por acción de los estrógenos, u hormonas sexuales femeninas, y puede llegar a alcanzar un tamaño enorme.
Más de un tercio de los miomas no dan síntomas. Los síntomas iniciales pueden ser cualquiera de los siguientes:
sensación de plenitud o presión en el bajo abdomen.
hemorragia o flujo no relacionados con la menstruación,
dificultad o dolor al orinar,
dolor en el área de la pelvis,
aumento del perímetro (tamaño) abdominal.
La cirugía es el tratamiento mas común del mioma uterino. Se suelen emplear dos tipos de cirugía:
1. Miomectomía. Extirpar sólo el mioma. Puede hacerse en tumores únicos y accesibles, en mujeres en edad fértil, y es deseable por cuanto se conserva la matriz.
2. Histerectomía, una operación en la cual se extrae todo el útero. Es necesaria cuando el mioma es múltiple, poco accesible, o cuando la conservación del útero no es imprescindible (después de la menopausia, por ejemplo).

A myoma is another term for a fibroid. These are almost exclusively benign growths and only need treatment if you are having symptoms. The most common ones are heavy or irregular vaginal bleeding and pain.
There is a surgical procedure called myomectomy which can remove the fibroids and preserve the uterus. It is best performed when fertility wants to be maintained.
The chances of getting pregnant after the surgery depend on many factors: your age, the location of the removed fibroids and any scarring that may form from the surgery.
An infertility specialist or an GYN with experience in this area can provide more throrough counselling. |

Paciente gestante a término, que llega a la emergencia el domingo 11 de Mayo de 2008 a las 16:00 hrs (día de la madre en Perú) con hemorragia vaginal. La Presión arterial es 85/60 mmHg y taquicardia. Al examen vaginal, dilatación de 4 cms, con cuello uterino engrosado. Inmediatamente se le practica ecografía pélvica y se descarta placenta previa. Los latidos cardiacos fetales se detectan en 144 x minuto y logramos notar un desprendimiento placentario de 5 cms de largo, causa de la hemorragia vaginal. Se opta por la aceleración del trabajo de parto con misoprostol 200 mcg, una tableta oral y Cloruro de Sodio parenteral con oxitocina 10mg ampolla.En el video observamos el parto, a las 17:00 hrs un neonato de sexo masculino de 3100 gramos y 47 centímetros de altura. Apgar de 10. Todo un milagro de Dios!!

The cardiac fetal beats detect in 144 x minute and the diagnostic was "abruptio placentae" of 5 cm long, cause of the vaginal hemorrhage. We decided in favour of the acceleration of the final stage labor vaginal with misoprostol mcg, oral tablet and Chloride of Sodium parenteral with "oxitocina" 10mg into of parenjteral cloruro. In the video we observe the childbearing, at 17:00 hrs a neonato of masculine sex of 3100 grams and 47 centimeters high. Apgar of 10. The whole God's miracle!! |
